Maryland Home & Community Care Foundation is dedicated to improving the quality of life for those in need of health and supportive services at home and in the community, regardless of age, by providing grants to organizations in Maryland.
The Foundation, a private non-profit corporation, supports new initiatives, demonstrations and programs in Maryland to include:
- Education and training grants to meet the needs of individuals, families and caregivers
- Educational programs for improving staff skills in initiating, developing, delivering and evaluating health and supportive programs
- Health and supportive services for vulnerable individuals in the community

- New or experimental approaches reaching individuals with unmet or underserved health needs
